The Family Behavioral Health System provides an integrated and flexible array of strengths-based therapeutic services and supports to families and children with complex therapeutic needs. Family Behavioral Health Services are delivered primarily in the client’s home or community and are designed to reinforce the strengths of the family to prevent the out-of-home placement of a child or prepare for the return home of a child from out-of-home care. Services focus on empowering the family to effectively use their strengths and abilities to address their needs and challenges with the primary goals of supporting them to become self-sufficient and to strengthen their relationships with community and natural supports that can sustain their long term success.
The Community Support Coordinator provides one-on-one treatment and support to high-risk children and youth in their homes and community. This includes the provision of medically necessary services and strategies as specified by the Clinician on the Treatment Plan and Safety Plan. Services and supports may include supervision for safety for high-risk youth, implementation of therapeutic activities or strategies (e.g. Cognitive Behavioral Therapy strategies per the treatment plan), assistance in accessing and safely participating in community activities, and mentoring and coaching positive social skills. The CSC work will follow a plan of care for each child and family under the direction of a Clinician. Hours are scheduled in a flexible manner based on program, agency and family need, with no minimum hours guaranteed.

Essential Functions: • Prior to introduction to the youth and family, participate in individual Treatment and Safety Plan development upon request
• Receive assignment from referring Clinician or QMHA Supervisor. If unclear, request additional information pertaining to strategies to be implemented
• Upon arrival to the home or community location, introduce role and describe activity plan for the day, engaging the youth and parent/family members in a way that communicates ‘friendly enthusiasm’
• While in the home and community with the youth, continuously assess safety and risk factors visually and report concerns to the Clinician or QMHA Supervisor within appropriate timeframes (e.g. When health or safety is at risk, contact supervisor immediately. For minor concerns, report at the end of the session or the following day).
• Adapt and modify strategies and activity ‘in-the-moment’ to fit the immediate situation and/or in response to family feedback, remaining as relevant as possible to the Treatment and Safety Plan goals and strategies
• Provide positive therapeutic structure and support throughout activity
• Model and teach appropriate behavior and skills
• Provide positive direct supervision and redirection of children and youth
• Model appropriate solution-based intervention skills for parents and caregivers
• When possible, proactively identify situations that require other staffing supports/team members to assure the most effective and appropriate response
• Anticipate behavioral escalations or crises and redirect or distract when possible, implementing strategies as specified on Safety Plan
• Call for assistance of supervisor and other team members at any time 24/7, if escalations continue or appear unsafe
• Continue to work to de-escalate and manage situations until other responders can arrive
• Focus on and verbally reflect successes experienced during activity or time with the youth and family members - communicate enthusiasm about progress
• Connect and introduce youth/family to community resources in their own community, (when identified as a strategy on the Treatment Plan)
• Identify and access positive natural supports and help the youth and family to strengthen relationships with such supports
• Ask the youth and family for feedback at the close of the session when appropriate (was this helpful today?)
• Communicate and summarize to the family results of completed activities and observations (I noticed James was working hard to practice his new conflict resolution skills with his brother)
• Communicate and summarize to CCS Team Members results of completed activities and observations within expected timeframe
• Accurately complete documentation per required timeframes, to be entered into the FBH Electronic Health Record, PsychConsult
• Other duties as assigned.
• Bachelor’s degree in social services or 3 years combined education and experience in social work.
• Age 21 or older per DHS requirements.
• Meets QMHA requirements per OAR 309-032-1505(106)
• Requirements for QMHA are defined under the current OAR as either A or B:
1. Experience: A combination of three years’ experience, work, education or training revolving around child mental health, child development and/or social skill development.
2. Education: a Bachelor’s degree from an accredited college or university with a major in the behavioral sciences field AND communicate effectively, understand mental health assessment, treatment and service terminology and to apply the concepts, provide psychology skills development, and implement interventions as assigned on an individual plan of care.
• Demonstrated ability to provide skill building services to youth.
• Must have reliable transportation, valid driver’s license, and auto insurance.
• Must have an acceptable driving record per CCS’ driving policy (required to provide current driving abstract).
• Applicant must successfully pass required background checks prior to an offer of employment.
• Availability to work flexible hours (evenings, nights, weekends).
• Demonstrate effective verbal and written communication.
• Ability to work both independently and as a member of a team.

• Demonstrates the necessary attitudes, knowledge and skills to deliver culturally competent services and work effectively in multi-cultural situations.
