The Macdonald Residence Assisted Living, located in the Oldtown neighborhood of downtown Portland has an opening for a Full Time RN. The Macdonald Residence is a 54 unit ALF serving our population alongside local nonprofit, Maybelle Center for Community www.maybellecenter.org. For the past 25 years, we have worked to help break isolation in this community. The Macdonald Residence is licensed by the State of Oregon and through our Specific Needs Contract we serve individuals whom often have a difficult time being accepted into other community-based care facilities.We strive to provide a safe, inclusive environment where our residents can thrive.ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned from time to time.· Assist in managing all aspects of the nursing department.· Assists with instrumental activities of daily living, ADL’s, environmental orientation, assistance with medication, treatments and other care while encouraging self-care and independence.· Provides services as indicated on resident treatment and service plans.· Performs new assisted living resident admission assessments prior to admission and determines appropriateness of the admission based on the needs of the resident.· Performs a full body assessment on each assisted living resident upon admission within 7 days of admit.· Manages the Negotiated Services Agreements system for all assisted living residents in the building in accordance with state guidelines.· · Performs on-going assessments of each assisted living residents physical and psycho-social needs and coordinates with dietary, activities, pharmacy, physicians and other disciplines to assure quality care.· Manages the self-medication assessment system in accordance with state guidelines.. Performs care conferences for residents and families when directed.· Acts as a liaison between assisted living resident, family, and physician to coordinate the care needs of each resident.· Supervises the maintenance of assisted living resident’s charts and reviews documentation performed by staff.· Responds to urgent situations as they arise based on nursing standards.· Monitor’s the weights of each assisted living resident and identifies significant weight losses.· Assesses each resident with significant weight loss at least once per month until resolved.· Assist in ensuring that all staff receives their T.B. testing per state guidelines.· Will assess residents with skin breakdown or skin at risk every 2 weeks.· Will initiate home health services for residents as needed and act as liaison between resident, home health agency, and physician regarding those services.· The Nursing Services Manager will be responsible for following the regulations and guidelines as set forth by the state of Oregon.COMPANY WIDE RESPONSIBILITES· Supports a dignified and caring atmosphere with residents, residents’ families, visitors and staff.· Protects privacy and confidentiality of information pertaining to the resident, employee, residence, company information and records.· Maintains a safe and secure working environment and practices safe working habits.· Complies with the attendance policy.· Maintains neat appearance, good personal hygiene and appropriate attire.· Participates in training, in services, and attends meetings as required.· Assists with projects as assigned and performs other related duties.SUPERVISORY RESPONIBILITIESShares responsibility for care staff.QUALIFICATIONSTo perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Must have a minimum of 2 years experience working with geriatric residents. Basic typing and PC knowledge is helpful.LANGUAGE SKILLSAbility to read, and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or employees of organization. Ability to effectively communicate in English and understand and follow written and oral instructions.MATHEMATICAL SKILLSAbility to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals.REASONING ABILITYAbility to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists.
